A 5 mg vardenafil start after the QT check

Rhythm drugs close vardenafil before onset even starts. Congenital long QT, quinidine, procainamide, amiodarone or sotalol: pick another PDE5 inhibitor. Helion's start lock is vardenafil 5 mg. The labelled usual start is 10 mg for many men. Five milligrams is how this site talks age, liver, alpha-blockers and caution. Compare clocks on the three-way page.

QT first, then the 5 mg start

The QT pass, then the class holds

Helion will not start vardenafil until the rhythm list is read. The label tells patients with congenital QT prolongation, or on class IA or class III antiarrhythmics, to avoid the tablet. That filter is what makes this page different from a sildenafil timing note.

Helion locks vardenafil 5 mg as the start we discuss. The US label's usual start is 10 mg about 60 minutes before sex, then 20 mg or 5 mg by effect. Five milligrams is already the written start for age 65 and over, moderate hepatic impairment, and men on a stable alpha-blocker.

If the QT screen is clean, vardenafil is a short clock: peak in under an hour for many men, half-life about four to five hours, once daily at most. It is the cousin of sildenafil with less PDE6 traffic and a sharper labelled rhythm warning.

Nitrates still ban it. A clean ECG does not reopen a GTN spray. Fit means both holds are clear, not only the QT line.

Headache, flushing and rhinitis were the common trial effects, with a dose trend from 5 mg to 20 mg. Back pain sat near placebo. That profile is why some men prefer it after a sildenafil tint, once QT is clear.

Staxyn 10 mg is another product

The orally disintegrating tablet (Staxyn and generics) is a 10 mg strength placed on the tongue without water. Systemic exposure is higher than after a 10 mg film-coated tablet. Milligram-for-milligram swaps are a mistake.

Helion's lock is the swallowed 5 mg film tablet. If a pharmacy hands over an ODT, that is a formulation change, not a convenience upgrade. The prescriber retitrates.

ODT labelling still carries the QT avoid-list and the nitrate ban. Dissolving on the tongue does not dodge rhythm drugs.

Some pharmacies substitute because film vardenafil is thinner on the shelf than sildenafil or tadalafil. Ask which product is in the bag before you leave. The 'no water' instruction is the giveaway that you no longer have Helion's 5 mg film lock.

Do not cut an ODT to approximate 5 mg. The wafer is a 10 mg unit. A home split is an unlabelled dose.

Diabetes does not pick the INN by itself

Vardenafil has dedicated trials in men with diabetes-related ED, including type 2, with better intercourse success than placebo. Sildenafil and tadalafil also work in that group. Diabetes is a reason to treat, not a reason to insist on 5 mg.

Neuropathy plus endothelial disease makes oral response less reliable. A fair short-clock trial still needs the hour and a light plate. If two INNs fail after that, the next step is the work-up and the non-oral rungs, not a third silent 20 mg at midnight.

Renal disease often rides with diabetes. Vardenafil still needs the usual organ caution. Tadalafil daily may already be vetoed by CrCl. Read the labs before you pick a cousin.

The QT pass still comes first. Diabetes plus amiodarone is a vardenafil stop, however good the diabetes trials look.

Autonomic neuropathy can blunt the nitric-oxide signal the tablet needs. A flat trial in long diabetes is often a nerve-plus-vessel file. Name the diabetes duration when you report '5 mg did nothing.'

What the thorough QT study actually showed

A crossover study in 59 healthy men compared vardenafil 10 mg and 80 mg with placebo and with moxifloxacin 400 mg. QT was read near one hour, around peak concentration.

Therapeutic and supratherapeutic vardenafil produced QTc increases in the same small band as the moxifloxacin control. A later study that paired vardenafil with another QT-active drug showed an additive effect. The label's 'avoid' list follows from that additivity, not from a story about everyday palpitations.

A thorough-QT design that also ran sildenafil found small QTcF shifts for both drugs. One report put vardenafil 10 mg near 8 ms and sildenafil 50 mg near 6 ms. Sildenafil and tadalafil still lack the same avoid-sentence for congenital long QT and class IA or III drugs. That sentence is why we screen vardenafil.

Uncorrected low potassium or magnesium lengthens QT on its own. So do some antibiotics and antipsychotics. The vardenafil decision is a list problem, not a single-tick ECG souvenir from five years ago.

Moxifloxacin was the positive control in the thorough-QT study. That does not mean every man on a quinolone must avoid vardenafil. It means additive QT drugs belong on the same scrap of paper as amiodarone when you decide.

When the visual tint made sildenafil fail

The man who got a useful erection on sildenafil and hated the blue wash is the cleanest vardenafil candidate, provided QT is clear. Selectivity for PDE5 over PDE6 is the chemistry reason that swap is rational.

Diabetes-related ED has dedicated vardenafil trials with better intercourse success than placebo. That does not make it uniquely better than tadalafil or sildenafil in diabetes. It does mean the short clock is a documented option in that group.

Age 65 and over is already a labelled 5 mg start. Moderate hepatic impairment (Child-Pugh B) is the same 5 mg start, with a 10 mg ceiling. Severe hepatic disease is outside casual titration.

If the week is spontaneous and dinner is never light, vardenafil is the wrong cousin. Tadalafil 20 mg ignores food and stays into the next day. Fit includes the calendar, not only the ECG.

Circulation reviews note no known torsade signal in the PDE5 class so far. That is not a reason to ignore the avoid-list. It is a reason not to frighten men with clean lists away from a useful short clock.

Colour-critical work plus a clean QT file is the other clean fit. The blue wash is less common. The short evening remains. If leftover Saturday is the real wish, say so before anyone writes 5 mg.

Onset like sildenafil, not like tadalafil

Dose about 60 minutes before sex. Median Tmax is in the same one-hour neighbourhood as sildenafil. Earliest reported onsets sit near 15-25 minutes for some men. Plan on the hour until you know your own curve.

The useful window is a few hours, not a leftover Saturday. If the week needs a 36-hour band, that is tadalafil 20 mg, not a higher vardenafil tablet.

Sexual stimulation is required, same as the rest of the class. A 5 mg start in a quiet room is a blood level. It is not an erection.

Once per day. A second 5 mg at midnight does not repair a late first dose. Strong CYP3A4 inhibitors collapse the ceiling hard: ritonavir means 2.5 mg and a 72-hour gap; several other antifungals and protease inhibitors cap 2.5 mg in 24 hours.

Ritonavir with vardenafil 5 mg raised AUC about 49-fold and Cmax about 13-fold in the interaction study, and stretched half-life to about 26 hours. That is why the 72-hour gap exists. It is not a polite suggestion.

Erythromycin is a milder example: about a 4-fold AUC rise with 5 mg, and a labelled cap of 5 mg in 24 hours. Bring the antibiotic name, not 'I was on something for a chest infection.'

Food is allowed; fat can still slow the peak

The film-coated tablet may be taken with or without food. That sentence is on the label. It is not the same as 'fat never matters.'

A high-fat breakfast study cut vardenafil Cmax in a wide band (about 18-50% in reviews) and delayed Tmax by about an hour. Manufacturers have treated that as less decisive than sildenafil's 29% / 60-minute pair. In clinic, a heavy dinner still makes a 60-minute plan optimistic.

If the tint on sildenafil was the reason for the switch, do not recreate the false-negative by pairing 5 mg with the same steak. Light plate, hour of lead time, then judge the INN.

Heavy drinking still drops blood pressure. So does an alpha-blocker if both starts were rushed. The 5 mg lock exists partly so those overlaps stay small.

Stable alpha-blocker therapy is a labelled 5 mg vardenafil start. Unstable, freshly titrated alpha-blockade plus a first 10 mg tablet is how a night faint happens. Stabilise one drug, then add the other.

Sign only after both holds

A 5 mg start is a fit when the rhythm list is clean, nitrates are absent, and a short, same-night clock is what the week needs. It is a poor fit when the calendar wants leftover Saturday or when amiodarone is already on the blister.

If sildenafil 25 mg failed only because of dinner, fix the dinner before you change INN. If it failed because of PDE6 tint, vardenafil is the rational cousin after the QT pass.

Priapism past four hours, sudden vision loss, sudden hearing loss: stop and get seen. Those class warnings did not retire when Levitra's brand box left the shelf.

If the 5 mg start is clean and weak after two timed nights, the labelled step is 10 mg, then 20 mg. Do not invent a 15 mg split. Do not add a second 5 mg at midnight.

If the only complaint after a clean QT pass is leftover Saturday, you picked the wrong clock. Switch to tadalafil 20 mg rather than chasing a long window with more vardenafil.

Vardenafil mail on this page is a fit question: QT, then a 5 mg start, then a short clock. I do not treat Staxyn as a 10 mg film tablet with extra mint. These four replies are teaching. A cardiologist's list still beats a urologist's preference when the QT line is in play.

I take sotalol. Can I still use a low 5 mg vardenafil tablet?

No. Sotalol is a class III antiarrhythmic. The label says men on class IA or class III drugs, or with congenital QT prolongation, should avoid vardenafil. Five milligrams is not a loophole. The caution is about additivity, not about a high starting dose.

Sildenafil 25 mg or tadalafil 20 mg are the usual other clocks, and they still need a nitrate check. Your rhythm prescriber and the person writing the ED tablet should see the same list. Do not trade leftover tablets with a friend who 'uses Levitra.'

If sotalol is stopped later, that is a new file, not a green light you award yourself the same week. QT drugs have tails. Let the cardiology side say when the avoid-list no longer applies. Five milligrams does not wait that conversation out.

He got a blue wash on Viagra 25 mg. Is 5 mg vardenafil the automatic swap?

It is a rational swap if the QT screen is clean and nitrates are absent. Vardenafil is more PDE5-selective and usually kinder to retinal PDE6. That is the chemistry reason the tint often settles.

It is not automatic if dinner was the real problem. A fatty meal delays sildenafil by about an hour. Try one properly timed 25 mg night on a light stomach before you change INN. Otherwise you may 'cure' a food delay by switching drugs.

If the week needs leftover coverage, skip the cousin and go to tadalafil 20 mg. Vardenafil will not give you a 36-hour band. It will give you another short evening.

The pharmacy gave Staxyn 10 mg instead of film 5 mg. Same thing?

No. The ODT is a 10 mg product with higher exposure than a swallowed 10 mg film tablet. It is certainly not a 5 mg film tablet in a melt form. Treat it as a different prescription.

Do not cut the ODT to 'make 5 mg.' The labelled unit is the whole 10 mg wafer, taken without water. Call the prescriber and the pharmacy before the first dissolve. QT and nitrate holds still apply.

If the substitution was silent, that is a safety issue, not a convenience win. Bring the box. The imprint and the 'no water' instruction tell you which product you actually have. A 5 mg film tablet is swallowed with water. An ODT is not.

He is 68 with Child-Pugh B. Is Helion's 5 mg lock the labelled start?

Yes on both counts you named. Age 65 and over is a labelled 5 mg consideration. Moderate hepatic impairment is a labelled 5 mg start with a 10 mg ceiling. That is why this site talks 5 mg rather than the 10 mg usual start.

Severe liver disease is not a home titration. CYP3A4 inhibitors on top of Child-Pugh B can force 2.5 mg and long gaps. Bring the full antiviral or antifungal list, not only the ED request.

Nitrates still close the visit. A careful hepatic start does not legalise a GTN spray. If angina treatment needs nitrates, we are not picking among 5, 10 and 20 mg. We are leaving the class. Child-Pugh B plus a nitrate is two holds, not one careful 5 mg.
